How to refer

Information for GP's/Health Care Professionals

We prefer to start patients on our programmes as soon as possible.  Patients can be referred to us by their GP, Practice Nurse, Hospital Consultant, local hospital or tertiary centre. Ensure patients understand what they have ‘signed up for’

Cardiac Rehabilitation

Please complete the following referral form GP Generic Referral Form and send to encrypted e-mail address [email protected]

Pulmonary Rehabilitation 

Please complete the following form Pulmonary_Referral_form_2019.doc and and send to encrypted e-mail address [email protected]

Long Covid, Long Term Conditions & Keep It Healthy 

Please use the GP Generic Referral Form and e-mail to [email protected]

Cancer

Please use the Cancer Referral Form to [email protected] or [email protected] along with any supporting clinic letters or information

What to do in an emergency

If you experience any chest tightness/discomfort/pain, or symptoms similar to your Heart Attack/previous Angina, please follow 1. If your Chest Pain is unbearable Dial 999 for an ambulance Otherwise – • Sit down and stay calm • If the pain does not ease after 30-60 seconds Take GTN (spray or tablets) • Wait 5 minutes • If the pain has gone – REST • If not repeat the GTN •Wait 5 more minutes AFTER 10 MINUTES AND TWO GTN SPRAYS - IF YOU ARE STILL IN PAIN DIAL 999 Read more
